Vaccinations

This guide can help you understand the vaccines offered in the UK and when to have them. It also explains how they work and why they're safe and important.

Continence assessment

Continence and prolapse problems are very common, but often men and women do not take steps to resolve these problems, assuming nothing can be done.

Social Prescribing

Social prescribing empowers people to take more control of their own health and wellbeing by connecting people to local community groups and statutory services for practical and emotional support.

Social prescribing can help to strengthen personal resilience and reduces health inequalities by addressing the wider determinants of health, such as loneliness and isolation, physical inactivity or living well with a health condition by increasing people’s connections with their local communities. It particularly works for people with long-term conditions (including support for mental health), for people who have complex social needs which affect their wellbeing.

If you would like any further information please contact Lauren Stone at the surgery.

Carers Support

If you provide care to someone who needs help because of their illness, frailty, disability, a mental health problem or an addiction and they couldn’t manage without your support then you are a carer.

Clinical Pharmacist

Clinical pharmacists and Pharmacy Technicians are increasingly working as part of general practice teams. They are highly qualified experts in medicines and can help people in a range of ways. This includes carrying out a variety of health reviews for patients with ongoing health problems and improving patient safety, outcomes and value through a person-centred approach. Our PCN Pharmacy team look at maximising benefits while minimising risks associated with medications.

Having clinical pharmacists in GP practices means that GPs can focus their skills where they are most needed, for example on diagnosing and treating patients with more complex conditions. This helps GPs to manage the demands on their time.
