Care Co-Ordinator Puddletown Surgery


18.5 hours a week

Salary up to £24,157 pro rata depending on experience (Band 4 equivalent).


Puddletown Surgery is a small rural training practice with 4,200 patients and three GPs. We provide dispensing services to 99% of our patients. We take pride in supporting our patients in sickness and in health.


Our integrated nursing team is unusual in that our nurses provide services to patients both in the surgery and in patients’ own homes, including work traditionally done by Practice and

District Nurses.


Care co-ordinators play an important role to proactively identify and work with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services.


They work closely with GPs and the practice team, acting as a central point of contact to ensure appropriate support is made available to them and their carers; supporting them to understand and manage their condition and ensuring their changing needs are addressed.


The successful candidate will be based at Puddletown Surgery, which is part of Mid Dorset Primary Care Network (PCN). They will be caring, dedicated, reliable and person-focused and enjoy working with a wide range of people. They will have good written and verbal communication skills and strong organisational and time management skills. They will be highly motivated and proactive with a flexible attitude, keen to work and learn as part of a team and committed to providing people, their families and carers with high quality support.
